Proxy Access (FERPA Release)

What Proxy (FERPA Release) Form Authorizes

The Proxy (FERPA Release) form only gives permission to share a student’s educational information. It allows the person named on the form to:
  • Review and inspect the student’s educational records.
  • Receive information from those records (such as grades, disciplinary records, or financial aid status).
  • Discuss the contents of the records with school officials. 

What a Proxy (FERPA Release) Does Not Authorize

The form does not give the proxy of legal authority or ability to act on the student’s behalf. The proxy may not:
  • Enroll the student in courses.
  • Request drops or withdrawals.
  • Act for the student in academic or disciplinary matters.
  • Use or access the student’s email account.
  • Submit student-related forms, including:
    • Transcript Requests
    • Student Data Change forms that change students biographical or contact information
    • Prerequisite waiver requests
    • Any other forms that require student action.
