The Residency Core Questions form is available in Adobe Acrobat PDF format. You will need Adobe Acrobat Reader to view and print these documents. The Acrobat Reader is available free for download from Adobe.
Changes MUST be requested prior to the census date of the current term in order to be considered for the current term, otherwise changes will be applied the following semester.

Since the Residency Affidavit must be notarized, it is only available in PDF format. Those needing to submit the Residency Affidavit should print it, complete the form, have it notarized, and mail it to the address listed on the form or drop it off to the Office of Student Admissions located in the Enrollment Services Center.
